How It Works – The Basics:
You commit to the following for 2 years
1. Come in for regular cleanings
2. Follow our recommended plan of treatment: Cavity Control > Bite Restoration > Esthetics
3. Pay $30 per month and pay for dental services at the time of service

2. Develop a customized treatment plan to get and maintain your mouth in a healthy, functional, and beautiful state

Frequently Asked Questions:
Q: Is there a family plan?
A: A single family, consisting of up to 2 guardians and any dependents living in the same house, will only pay a maximum of $150/month, the cost for 5 individual participants. The 6th family member and beyond will not incur any addition monthly charge.
Q: Do we offer payment plans?
A: We do not offer payment plans under the CodySmiles Initiative. Payment for all treatment is required at time of service. However, we do accept CareCredit and Cherry. We will be happy to help you apply for these services. We do add a 10% fee for all payments made via CareCredit or Cherry.
Q: Do you treat children?
A: We will complete cleanings for children of all ages. We will only routinely complete dental work on children ages 10 and above. Children younger than 10 are usually referred to a pediatric dentist for needed dental work. We strive to provide positive dental experiences for children and all patients, however, our office is geared toward treatment of older children and adults. Often our office does not provide the optimal setting for younger children. As with patients of all ages, if children are unable to cooperate and tolerate needed treatment, they will be referred.
Q: What dental procedures are including in the CodySmiles Initiative?
A: All procedures that can be completed in our office at the standard of care or higher are included in the Initiative. These include but are not limited to fillings, crowns, root canals (excluding molars), implants, dentures (complete, partial, over and hybrid), bone grafts, all types of cleanings, extractions, Invisalign (esthetic and simple cases only), etc.
Q: What happens if treatment is unable to be completed in our office?
A: You will be referred to a specialist like an oral surgeon, orthodontist, periodontist, or endodontist who can complete the needed treatment. Any treatment not complete in our office is subject to the fees charged by the specialists.
